As health care provider, we collect, use and disclose personal information to serve our clients.  We may collect your personal information directly from you or from the person acting on your behalf.

What is Personal Information?

Personal information is information about identifiable individuals. Personal information that relates to:

  • an individual’s personal characteristics (e.g., gender, age, home address or telephone number, family status);
  • health (e.g., health history, health conditions, health services received by them);
  • activities and views (e.g., opinions expressed by an individual, an opinion or evaluation of an individual).

Personal information is different from business information (e.g., an individual’s business address and telephone number). This is not protected by privacy legislation.

Uses and Disclosures of Personal Information

Lily B. Well may use and disclose your personal information for the following purposes:

  • Notify clients of appointment changes, reminders, to follow-up and specials via email
  • Treat and care for you
  • Receive or directly invoice for your treatment and care
  • Conduct risk management and quality improvement activities
  • Comply with legal and regulatory requirements
  • Fulfill other purposes permitted or required by law

Your Rights

You may access and correct your personal information records or withdraw your consent for some of the uses and disclosures (subject to legal exception) by contacting the clinic administration.

Our Responsibilities

I understand the importance of protecting your personal information. For that reason, Lily B. Well have taken the following steps:

  • Paper information is either under supervision or secured in a locked cabinet.
  • Computers and backups are under supervision or secure in a locked or restricted area at all times. In addition, passwords are used on computers, and screens are locked when they are not in use.
  • Electronic information is transmitted either through a direct line, or has identifiers removed, or is encrypted.

Retention of Personal Information

We are required to retain personal information for some time to ensure we can answer any question you may have about the services provided and for our own accountability to the College of Massage Therapists of Ontario (CMTO).

Client Files are retained at Lily B. Well for 10 years as required by the CMTO.

We destroy paper files containing personal information by shredding and deleting electronic information. When hardware is discarded, we make sure it is destroy properly.
